A new Shipper's Export Declaration (SED) form approved by the U.S. Census Bureau is now available. The revised Form 7525-V is designed to eliminate unused or outdated data fields, update current federal government agency names, and agree with data elements on the AES record.

New form features include fields of information for carrier, identification code, shipment reference number, entry number, hazardous materials, in bond code, and routed export transaction; required vehicle identification information; and space for a telephone number and email address.

This company has SED forms available in carbonless versions with four- or five-part snap sets or five-part continuous forms, and in carbon sets with four-, five-, six-, or seven-part snap sets or five-part continuous forms. In addition, they provide a free instruction card with each order to help users accurately complete the new forms.

The new SED forms became effective for use October 1, 2000 and will be mandatory on April 1, 2000.
